At TMC Rehab, we offer a full continuum of care to support every stage of recovery. Our programs are designed by experienced clinicians and personalized to meet individual needs.
Medical Detox
From the moment you arrive at our Long Beach facility, every aspect of detox is delivered in the language you think in. Our bilingual clinical team provides three to seven days of around-the-clock medical care, ensuring that medication explanations, consent conversations, and comfort check-ins all happen without a language barrier. Withdrawal management is shaped by the specific substance, your medical history, and any co-occurring psychiatric needs — because culturally responsive care starts with understanding the whole person, not just the diagnosis.
Every patient receives an explanation of their detox protocol in their primary language at admission and at every medication transition — not as a courtesy, but as a clinical standard.
What's included: 24/7 nursing, physician-managed medication, psychiatric consult within 48 hours, MAT when clinically appropriate, trauma-informed bedside practice, and cultural-humility check-ins at shift change.

Residential Treatment
A 30 to 90 day stay in our 28-bed residence at 2712 East 4th Street. Days are anchored by a morning community meeting, a peer mentorship pairing from week one, and a structured evening house meeting — a rhythm that research consistently shows strengthens long-term outcomes.
Residential care here pairs clinical work (individual therapy, group, family sessions) with the embodied practices addiction recovery research supports: nutritional psychiatry, mindfulness-based relapse prevention, music therapy, and optional spiritual-care track for patients who want one.
What's included: private rooms, gourmet meals designed by our registered dietitian, access to the meditation labyrinth and walking trails, family lodge for weekend visits, and case management for the transition home.

Outpatient Program
Three outpatient tracks built for working adults, parents, and caregivers across Long Beach, Signal Hill, Wilmington, San Pedro, and the wider Harbor Area.
PHP runs 5 days per week, 6 hours per day, for 2–4 weeks. IOP runs 3 evenings per week for 8–12 weeks — designed for shift workers, parents, and healthcare workers coming off duty. Standard outpatient continues weekly therapy and psychiatric medication management for 3–6 months after step-down.
Step-down between levels is determined by clinical milestones and patient-defined goals, not a fixed calendar.
Dual Diagnosis Treatment
Research consistently shows that more than 60% of patients with substance use disorders have a co-occurring psychiatric condition — depression, anxiety, PTSD, ADHD, bipolar disorder. Treating one without the other is the most common clinical error we see among patients arriving from other programs.
Our dual diagnosis track keeps psychiatric medication and substance use therapy under one coordinated clinical team. Dr. Tesfaye reviews every plan; the psychiatrist who prescribes is the same psychiatrist who attends the family meeting.
Approach: integrated assessment in week one, single coordinated treatment plan, trauma-informed intervention for PTSD-driven cases, cultural context honored throughout, and discharge planning with named local providers who continue the work.
